Health Promotion Disease Prevention
Healthy Living Messages: Be involved in your health care, be tobacco free, eat wisely, be physically active, strive for a healthy weight, limit alcohol, get recommended screening tests and immunizations, manage stress, and be safe.
The goal of the Health Promotion Disease Prevention (HPDP) Program is to give you, the Veteran, the tools to make healthy choices as an active member of your health care team. Erie VA offers a variety of HPDP programs designed to help you achieve your goals.
Healthy Lifestyle Programs
This is an online tool that helps patients manage their health care online. Patients can refill prescriptions, track their health information, view doctors notes, view lab results, and even send secure messages to their health care team. Go to www.myhealth.va.gov to register for the program. Once you register, stop into your clinic to sign a one-page in-person authentication form to upgrade your account so you have full access to all the services.

MOVE! Weight Management Program
This national weight management program helps Veterans lose weight, keep it off, and live healthier. There are many options to pick from for the MOVE! Program including teleMOVE (via the phone), support group meetings, and individual face-to-face meetings with a registered dietitian.

Tobacco Cessation Program
There are many programs offered to Veterans to support them in their efforts to quit using tobacco. Choices range from a 1-time educational group, weekly support group, Quit for Life telephone coaching, Stay Quit mobile app, SmokefreeVet mobile text messaging service, and an online resource: www.ucanquit2.org.

Women Veterans Program
This Veteran-centered program helps women Veterans navigate the VA system and connects them to a variety of unique programs specific for women.

Primary Care Mental Health Integration
This program offers brief solution-focused care for those who are experiencing mild to moderate symptoms of depression, anxiety, and grief. In addition a social worker is available to discuss mental health and substance abuse treatment options.

Behavioral Health Specialty Care
Specialty care is available to Veterans who are experiencing more complicated mental health symptoms related to depression, anxiety, post-traumatic stress disorder and more who would like to receive counseling.

Veterans Crisis Line, 24/7 Support
If you are experiencing an emotional crisis, the VA has established a 24/7 Crisis Line where you can speak to a trained mental health professional. This service is free and confidential.
